If you require a 1st, 2nd or 3rd class exam, please schedule a 15 minute time slot.

NOTE: If you are seeking a 1st class and you are 35, you will need to do an EKG baseline. If you are 40 you will need an EKG every year.

Complete the MED XPRESS form and bring the confirmation number.

NOTE: If you have a SODA or Special Issuance, remember to bring your documents with you including your PI number.

Regular ATC please bring in paperwork.

There is a $5.00 surcharge for payments processed via credit card.

There will be a $10.00 service charge for color vision testing if required.
